Overview

All our forms begin with a start page to let the customer know what they need to complete it and to give them an overview of

  • What the transaction is
  • How many pages or sections the form has
  • What information do they need to provide to complete the transaction

Each start page should have

  • The name of the service
  • A short introduction and service information
  • The number of pages or sections
  • An estimate of how long it will take to complete the form
  • A start button or ‘call to action’
  • Eligibility criteria

Things to remember

When you count the number of pages in a form, do not include the success or failure pages. If forms have differing numbers of pages depending on the answers given, change the message to ‘up to xx pages’.

Example

Book your appointment to register a birth

This form has 4 pages and will take approximately 5 minutes to complete

Before you start

You will need to know:

  • Where the child or children were born
  • The child or children’s date of birth
  • The mother’s full name
  • You can also book an appointment by phoning 0345 155 1002.

Eligibility

Everyone who needs to register a birth must make an appointment.
